Note that when the application windows disappear from the space in which you are using a program, hiding and then bringing the program back to the front solves the issue most of the time.

My experience is that the problem may be linked to when program notifications pop up in a different space than the one program is located in. For example you have Firefox in space 1 and are using a different app or even just hanging out in space 2, if an alert from Fx is issued it pops up in the active space (2). Dismissing the alert in the split space environment of an app confuses Leopard, which then displays the erratic hidden app syndrom.
