I found this article http://www.stuartcheshire.org/papers/NagleDelayedAck/ fully explained the underlying issues, "a TCP performance problem resulting from a little-known interaction between Nagle's Algorithm and Delayed ACK". It is a long standing issue relating to optimizing the TCP data packets, TCP segment size and ACKs.
