As far as I can tell, there's no difference between this and clicking the To Do button. Both create a new entry in the To Do folder.

If you do it using the to do button, the to do doesn't appear along with the mail. But if you right click on the message and make a new to do, it stays with the mail and whenever you click on it, you can see the to do on top of the mail.
